Peace of Mind!

T-Mobile provides you with life insurance, including accidental death and dismemberment coverage, equal to 1.5 times your annual base pay and commissions up to $1M.

You can choose to buy additional life and accidental death & dismemberment (AD&D) insurance for yourself, up to 8x your annual pay, at reduced rates and pay for it directly from your paycheck. If you have dependents, you can also purchase spouse/partner voluntary life, child(ren) voluntary life, and employee and family AD&D insurance at reduced rates.

Additional coverage you can purchase during annual enrollment

Employee or Employee and Family Voluntary AD&D Insurance	Up to 8 times current annual pay, up to $2 million For Employee and Family Coverage, spouse/partner and child(ren) coverage amounts are based on percentage of the employee election (please refer to page 7 of the Life and AD&D Benefit Highlight Sheet for details)	Employee-paid Premiums based on coverage level and amount elected.

Voluntary Life Insurance Costs

The cost for voluntary life insurance for you or a spouse/partner is based on age and coverage amount elected. The rates below are per $1,000 of coverage, per pay period. No need to do the math - the enrollment tool will calculate the cost for you!

Child(ren) Life Coverage: The cost for voluntary life insurance for your dependent child is $0.535 for $10,000 of coverage and $1.062 for $20,000 of coverage per pay period, regardless of their age or number of children enrolled.

Example: Your spouse is 31 and you're enrolling them for $75,000 of coverage. $75,000 in coverage divided $1,000 = 75. Multiply that by the rate for Spouse/Partner Coverage at age 31 ($0.038). Cost per pay period is 75 x $0.038 = or $2.85/pay period.

Voluntary AD&D Insurance Costs

The cost for voluntary AD&D insurance is based on the level of coverage (Employee only or Employee and Family) and coverage amount elected. The rates below are per $1,000 of coverage, per pay period. No need to do the math - the enrollment tool will calculate the cost for you!

Example: You enroll in 4 times AD&D insurance for yourself and your family and your annual pay is $50,000. $200,000 in coverage divided $1,000 = 200. Multiply that by the rate for Family Coverage ($0.007). Cost per pay period is 200 x $0.007 = $1.40/pay period.

Don't Forget! Assign Your Beneficiaries

Make sure your loved ones are taken care of. Update your beneficiaries any time for all of your Life and Accidental Death and Dismemberment insurance coverage(s) by logging on to the Enrollment Site.

Accelerated Benefit
