Coverage That Works for You

T-Mobile offers several flexible medical plan options, so you can choose coverage that fits your needs, your budget, and how you use healthcare. Review the options below to get started. You can contribute up to the IRS contribution limit each year.

Your Plan Type and Insurance Company

There are four plan types available. If you choose the Surest Copay Only Plan, your insurance company is Surest. For the HRA, EPO, and HSA plan, you can choose Premera Blue Cross or UnitedHealthcare.

Learn More About Prescription Coverage

Prescription drug coverage is included with every T-Mobile medical plan through CVS Caremark. Explore covered medications, compare prescription costs, find network pharmacies, and review how prescription benefits vary across plans before you enroll.

All of T-Mobile’s medical plan options include prescription coverage, virtual care, and access to tax-advantaged savings accounts—like HSA, HRA, and Flexible Spending Accounts (FSA)—so you can set aside money for eligible healthcare expenses and potentially pay less in taxes.

What to Know Before You Compare Plans

  • In-network preventive care is covered at no cost to you.
  • For most other care, you’ll share in the cost—but how you pay varies by plan.
  • You’ll pay less when you use in-network providers. The EPO Plan and Surest Copay Only Plan only cover in-network care, except in emergencies.

*The High Deductible Health Plan with HSA Plan features a true family deductible. The family deductible is two times the individual deductible and requires that the entire family deductible is met before paying 80% of covered charges. Please note that the out-of-pocket maximum works the same way.

Measuring Your Plan's Value

All of T-Mobile's medical insurance options are rated Gold under a commonly used healthcare standard established by the Affordable Care Act (ACA). This means all four plans are designed to pay a larger share of covered healthcare costs on average than Bronze and Silver plans commonly available through the Health Insurance Marketplace.

One way to compare plan coverage is actuarial value, which estimates the average percentage of covered healthcare and prescription drug costs a plan is expected to pay. T-Mobile's plans have actuarial values ranging from 85% to 88% for 2026 and from 86% to 89% for 2027, meaning T-Mobile is expected to pay most covered healthcare and prescription drug costs on average.

Surest Copay Only 88% 88%
PPO + HRA 88% 89%
EPO 88% 86%
HDHP + HSA 85% 89%

Maximize Your Healthcare Savings

Take advantage of tax-advantaged accounts to help your healthcare dollars go further. Healthcare Flexible Spending Account (FSA), Health Savings Accounts (HSAs), and Health Reimbursement Accounts (HRAs) can help you pay for eligible healthcare expenses with tax advantages—while potentially reducing your taxable income. T-Mobile may also contribute funds to help cover eligible expenses.

Bi-Weekly Costs

T-Mobile covers most of the cost of your medical coverage, helping keep your paycheck contributions lower.

Your bi-weekly (per-paycheck) cost is based on:

  • Medical plans
  • Who you cover
  • Working Partner Premium (WPP), if applicable
  • Insurance company (UnitedHealthcare vs. Premera Blue Cross)
  • Your location

Costs may vary by location based on provider discounts negotiated in your area.

For your most accurate bi-weekly cost, log in to the Enrollment Site to view your personalized rate and plan options.

You can also review the rate chart for general cost ranges by plan. Depending on the plan you choose, rates may be fixed or vary based on the factors noted above.

If you have a domestic partner or child of your domestic partner enrolled on the medical plan, the amount T-Mobile pays for your domestic partner/child of domestic partner coverage is added to your paycheck as taxable income because the IRS does not consider either to be a tax dependent. For additional information, please log in to your account.

Working Partner Premium (WPP)

An additional premium may apply if you enroll your spouse or domestic partner in a T-Mobile medical plan. For 2026, the WPP is an additional $46.15 per paycheck.

WPP applies if:

  • Your spouse or domestic partner has access to medical coverage through their employer, but waives that coverage and enrolls in a T-Mobile plan.

WPP does not apply if:

  • They also enroll in their own employer’s medical plan (that plan becomes primary coverage).
  • They are a T-Mobile employee enrolled under your plan.
  • They are not eligible for employer-sponsored medical coverage or are unemployed.
  • They are enrolled in or eligible for COBRA.
  • They are eligible for Medicare.
  • You do not enroll them in your T-Mobile medical coverage (enrolling them in other benefits such as dental, vision, or life insurance does not trigger WPP).
