Alfac Accident Insurance

Extra financial protection when an accident happens.

Accidents can happen when you least expect them—and the costs can add up quickly. Aflac Accident Insurance provides benefits for covered injuries and accident-related care, helping with expenses that may come with treatment and recovery.

The plan can help provide financial support for covered services and injuries such as:

Ambulance rides
Emergerncy room visits
Fractures
Concussions
Diagnostic Testing related to an accident
Crutches and wheelchairs

Wellness benefit - $60 once per calendar year

Features:

Benefits are paid directly to you, unless otherwise assigned.
Coverage is guaranteed-issue (which means you may qualify for coverage without having to answer health questions).
Benefits are paid regardless of any other medical insurance.

Updates for 2027:

Higher benefit amount payouts for emergency room, urgent care, and doctor’s office visits; ambulance services; diagnostic testing; fractures; dislocations; and lacerations; plus increased hospital admission, follow-up treatment, and therapy benefits.

A serious illness can bring unexpected costs at a time when your focus should be on your health. Aflac Critical Illness Insurance pays cash benefits for covered conditions, providing added financial support for medical costs, everyday expenses or other needs.

The plan provides benefits for covered critical illnesses such as:

Aflac for T-Mobile Employees

T-Mobile makes the following insurance plans available during annual enrollment, unless you are a new hire - new hires can enroll within 90 days of their hire date.

Group Accident Insurance

Helps pay costs that arise from covered accidents such as fractures, dislocations and lacerations. This plan provides 24-hour coverage, and includes a $60 Wellness Benefit for the covered employee, spouse and dependent child(ren), payable once per calendar year per insured.

Group Critical Illness Insurance

Helps pay the expected and unexpected expenses that arise from diagnosis of a covered critical illness such as cancer (internal or invasive), heart attack, stroke, end-stage renal failure or a major organ transplant. The plan also includes a $75 Health Screening Benefit (for covered employee and spouse only; not payable for dependent children). During the enrollment, Critical Illness is being offered on a guaranteed-issue basis (no health questions).

Group Hospital Indemnity Insurance

Helps pay the costs associated with a hospital stay including benefits for hospital confinement, hospital admission, hospital intensive care, or a rehabilitation facility.

T-Mobile Employees – Did You Know?

Aflac Group Accident and Critical Illness insurance plans offer benefits simply for taking advantage of wellness/health screening tests performed as the result of preventive care, including tests and diagnostic procedures ordered in connection with routine examinations.

Group Accident Wellness Screening Benefit

$60 once per calendar year

This benefit is payable once per insured, per calendar year for the covered employee, spouse and dependent children.

Group Critical Illness Health Screening Benefit

$75 once per calendar year

This benefit is payable once per insured, per calendar year for the covered employee and spouse only; dependent children are not eligible.

Any balance rolls over to the next year, up to $6,000 as long as you remain in the plan. T-Mobile contributions are prorated and funded by the Friday following each pay date. Any funds in your HSA are yours to keep, even if you leave T-Mobile. **The High Deductible Health Plan with HSA Plan features a true family deductible. The family deductible is two times the individual deductible and requires that the entire family deductible is met before paying 80% of covered charges. Please note that the out-of-pocket maximum works the same way.
