Evidence of Insurability (EOI) provides details about your health, the health of a spouse, or health of a dependent in order to qualify for certain Life, Disability, and/or Accidental Death & Dismemberment (AD&D) insurance coverages.

What is Evidence of Insurability?

Evidence of Insurability (EOI) is a record of a person's past and current health events. It's used by insurance companies to verify whether a person meets the definition of good health.

How to submit EOI?

If you’re required to submit Evidence of Insurability (EOI), you’ll receive a notification in My Tasks.
To get started:

  1. Visit the T-Mobile enrollment site
  2. Log in and navigate to the “I Want To” section.
  3. Select “Submit Evidence of Insurability”

Click “Complete Application” on the right side of the page. This will take you to The Hartford’s website, where you can finish your submission.
