Evidence of Insurability

Evidence of Insurability (EOI) provides details about your health, the health of a spouse, or health of a dependent in order to qualify for certain Life, Disability, and/or Accidental Death & Dismemberment (AD&D) insurance coverages.

What is Evidence of Insurability?

Evidence of Insurability (EOI) is a record of a person's past and current health events. It's used by insurance companies to verify whether a person meets the definition of good health.

How to submit EOI?

If you’re required to submit Evidence of Insurability (EOI), you’ll receive a notification in My Tasks.
To get started:

Visit the T-Mobile enrollment site
Log in and navigate to the “I Want To” section.
Select “Submit Evidence of Insurability”

Click “Complete Application” on the right side of the page. This will take you to The Hartford’s website, where you can finish your submission.

When is EOI required?

Employee Voluntary Life Insurance

You may enroll or increase your coverage by one level during Open Enrollment. Current plan participants are required to complete EOI if:

You have a Qualifying Event and enroll yourself or your spouse in Voluntary Life Insurance.
The amount exceeds the guaranteed issue amount of $750,000

Disability insurance

If you do not enroll in Buy-Up Long-Term Disability (LTD) plan when you are initially eligible (30 days from your date of hire) and then enroll later during Open Enrollment or a Qualifying Event, you must provide EOI to The Hartford.

Note: You must be actively at work for your Life, AD&D, and Disability coverages to begin. If you are not actively at work, coverage will begin when you return to active employment and have met the eligibility waiting period.
